Where should I record the date an order is needed?

Use the date-needed field for the customer's deadline and keep it separate from the order date. If you promise a pickup or delivery time, write that in the handover section so the two dates are not confused.

Does this form include room for deposits?

Yes. The payment area includes subtotal, deposit paid and balance due, along with a payment-method line. Enter only the amounts agreed with the customer and keep any extra payment conditions in your own terms.
